Six Sigma projects focus on improving processes that are producing substandard products or services. The goal of a successful Six Sigma project is to significantly improve the process to reach not more than 3.4 Defects Per Million Opportunities (DPMO).
With a Six Sigma approach, appropriate tools are used. The goal is to provide operational businesses or companies with valuable financial returns.

To apply for ASQ Six Sigma Certification Exam, the Six Sigma Project Affidavit Form needs to be filled up by those planning to obtain the ASQ Blackbelt or Masters belt Six Sigma.
To receive the approval for your Six Sigma Blackbelt Exam application approval, you need to satisfy two major requirements of ASQ:
1. Latest CV with a minimum of 3 years work experience related to one or more of the areas of the Certified Six Sigma Green Black Belt of Knowledge .
2. Signed Six Sigma Project Affidavit form – (at least one six sigma project for those who have 3 years work experience and two for having less than 3 years work experience)
1. Six Sigma Applicant Name and Applicant ASQ Member Number
2. Six Sigma Project Title
Example: Improvement of Customer Return-Repair-Replace Process Flow.
3. State the purpose of the project, and how it is related to the business objectives
Example: Customer complaint management processes were observed to be inefficient from a resolution time and brand reputation perspective. The aim of the project was to introduce remote troubleshooting processes and internal process improvement, tighter control over replacements, resulting in higher customer satisfaction and quality cost reduction. This was in line with the organization’s customer satisfaction and quality cost reduction goals.
4. Provide the Six Sigma project’s start and completion dates by month/year
Example: September 2020 – December 2020.
5. Include a brief description of the applicant’s hands-on performance in completing the Six Sigma project
Example: Project charter to document project scope and objectives, project plan to set a realistic timeline, brainstorming sessions with the team to understand challenges and ideas, process maps to document existing flows which were used as inputs in Pareto analysis, Voice of Customer and Voice of Business used as inputs to define the new flow, procedures to document steps and actions to be taken, control plan to ensure the system stays in place.
6. Provide the benefits achieved by the successful completion of the project including but not limited to financial savings, labor, material costs, cycle-time reduction, etc. Measured before-and-after benefits must be included. Examples
