How do I change my details on a Prescription Prepayment Certificate (PPC)?

Your personal details on your PPC must match your GP records to avoid a Penalty Charge Notice (PCN).

If you need to update your name or date of birth, you must send us evidence by email or post.

Once we've received your evidence, you'll be sent a digital certificate by email within 24 hours.

If you have a paper certificate, your new certificate will be sent by post within 10 working days.

We recommend sending photocopies. If you send us original documents, these will be returned within 20 working days from the date you sent them.

If your PPC shows the wrong name or you’ve changed your name, you must send us evidence of the correct name.

If you have a paper certificate, you must destroy it.

If you have any prescriptions to collect whilst waiting for your new PPC to arrive, tick ‘box F’ on the back of your prescription.

If your PPC shows the wrong date of birth, you must send us evidence of the correct date of birth.
