Important Update: With the opening of CGI Seattle, CGI San Francisco now serves the states of California, Arizona, Nevada, Utah, Hawaii, and Guam. Residents of Alaska, Idaho, Montana, Nebraska, North Dakota, Oregon, South Dakota, Washington, and Wyoming should submit their applications to CGI Seattle.
Applying In Person:
All miscellaneous attestation services are now accessible on a walk-in basis, without the need for an appointment, from 9:00 AM to 12:00 PM on regular working days.
Carry originals of all the supporting documents to be presented to the Consular Officer (photocopies of which are to be attached with the application). In case you are not able to present the original documents at the counter bring notarized copy of documents being attached to the application. Ensure that notarized copy is legible, otherwise Consulate may ask to see the original. However, the passport should be presented in original.
Delivery of Documents (Pick-up Hours) : 04.00 pm to 04.45 pm (Monday to Friday)

Photocopy of all supporting documents should be notarized and attached to the application. Do not send any original documents except the affidavit / document to be attested.
A non-refundable fee of $ 25 (twenty five) + $2 Indian Community Welfare Fee (ICWF) w.e.f. Oct. 01, 2012 per attestation.
Fees should be paid through Money order or Cashiers’ cheque drawn in favor of the ‘Consulate General of India, San Francisco’. NO BLANK CHECK/MONEY ORDER WILL BE ACCEPTED BY THE CONSULATE. Cash, Personal Cheques, Credit Cards, Debit Cards or other banking instruments are not accepted.
How to Apply:
- Miscellaneous Services Form - Download the Miscellaneous Application form and complete the application form.
- Attach the following documents with the application form.
- Notarized copy of Passport (First and Last Page)
- Notarized copy of any one of the following as proof of your US Visa Status:
o Page containing visa on passport (H1B, H4 etc) and the copy of I-94
o Clear Photocopy of Green Card
o Employment Authorization Document (Work Permit)
o I-797, I-140 or I-20 (If approval copy of these notices are pending, also attach a handwritten note detailing the efforts being made to regularize status)
- Notarized copy of any one of the following as proof of your current US residence address:
o U.S Driving license.
o Gas, Electricity, Water or landline telephone bill displaying applicant’s address
o House Lease Agreement
o State Identification Card
o Note: Bank/credit card/mobile phone statements are not accepted as residence proof.
